My uneducated guess right now is that the design and all procedural requirements with regard to the design were adequate. That is almost always the case. I would look primarily at underspec material and sloppy construction procedure. People in South Africa are increasingly using imported cement and steel from China - which are often not according to the specification against which it was delivered, and building procedure is crucial. Reinforced concrete improperly laid and cured is a timebomb. We are always justifiably dismissive of the intelligence of those involved with construction, but there must always be at least one person on site with at least half a brain.

Apparently the name of the structural engineer has not yet been disclosed. This building was originally planned (my understanding) as a three floor building which morphed into 4 floors and then into 5 and then a basement was added. Not necessarily in that order. Agree with all you say, but my understanding is that the engineer's input is taken at face value (ie when signed off, it is not checked). With all those changes, I would not discount the possibility of error on the design of the main structure.
