I would look primarily at underspec material and sloppy construction procedure. People in South Africa are increasingly using imported cement and steel from China - which are often not according to the specification against which it was delivered, and building procedure is crucial. Reinforced concrete improperly laid and cured is a timebomb. We are always justifiably dismissive of the intelligence of those involved with construction, but there must always be at least one person on site with at least half a brain.
